Case Manager – Corrections Transition Programs (CTP)
Location: Sheridan Correctional Center, Illinois
Salary: $44,676–$47,650 (Based on Experience)
Schedule: Monday–Friday, 7:30 AM–3:30 PM | This is not a hybrid position; in-office work is required daily

Position Overview: TASC is seeking a dedicated Case Manager for our Corrections Transition Programs (CTP) division located at Sheridan Correctional Center. This position plays a vital role in supporting individuals returning to the community after incarceration by offering specialized reentry case management. The ideal candidate will provide addiction intervention, AODA counseling, and recovery support services to clients and their families while coordinating access to critical resources.
Please note: Candidates must be comfortable working inside a prison environment.
Key Responsibilities:
Conduct comprehensive assessments and develop individualized service plans
Facilitate intake, enrollment, and coordination of services inside correctional facilities and in the community
Provide referrals and crisis intervention, including behavioral health, housing, public benefits, and employment
Collaborate with internal teams and external partners to ensure holistic and integrated care
Monitor and document client progress toward service goals, using systems like ASAM criteria and DSM-5
Complete accurate, timely records including progress notes, service plans, and discharge summaries
Assist clients in obtaining vital records and enrolling in public benefit systems
Attend team meetings, staffings, and community partner events to support client care
